By completing this form, I acknowledge that I am voluntarily registering my Emotional Support Animal in the United Service Dog registry. This registration provides identification materials intended to help others recognize my Emotional Support Animal.
I understand that in order to legally qualify for an Emotional Support Animal, I must possess a signed letter from a licensed Mental Health Professional or Physician stating my need for an Emotional Support Animal. I understand that such documentation may be required to be renewed annually depending on applicable laws, housing providers, or other entities.
I certify that I am not registering my animal for the purpose of fraudulent misrepresentation and that my dog must meet all applicable legal requirements for Emotional Support Animals in order for any protections or privileges to apply, including housing-related accommodations where permitted by law.
I acknowledge that federal, state, and local laws regarding Emotional Support Animals may change over time, and I agree to remain responsible for complying with all current laws and regulations.

I agree that United Service Dog provides identification materials only and does not guarantee access, privileges, or acceptance by landlords, airlines, businesses, or any other third parties. I agree to hold United Service Dog and its agents harmless from any disputes, denials of access, or related issues that may arise.
Any cancellation or return requests are subject to the company’s return policy.
Orders returned due to invalid addresses, relocation, or delivery issues are non-refundable and may be subject to a reshipment fee of up to $6.99.
I also authorize United Service Dog and its agents to contact me via phone, text message, or email regarding my order and to send occasional promotional communications.

Service Dog Agreement: I acknowledge that my completion of this form is to voluntarily register my Service Dog into United Service Dog's registry. Doing so will provide me with identification materials to help others identify my Service Dog. Furthermore, I certify that my Service Dog has been trained to assist me with my disability. I am not registering for fraudulent misrepresentation of a Service Animal, and understand that my dog must meet the legal requirements in order for this registration and identification materials to be valid and to receive the privileges, including access into housing and public places with a Service Animal. Additionally, I understand that both federal and local laws regarding Service Dogs are subject to change in the future, and I will continue to abide by all current regulations. I am aware that additional documentation may be needed to travel with my Service Animal that must be provided to airlines at least 72 hours before any flights. Should any access or privilege issues arise, I hold United Service Dog and its agents harmless, and understand United Service Dog can provide no guarantees that landlords or any other third parties will be compliant and non discriminatory with access and privileges protected by laws regarding services animals. Once submitted, customized items cannot be cancelled once printed, and requests for any cancellation are subject to our return policy. Orders returned due to invalid addresses or relocation are non-refundable and a subject to a reshipment fee of up to $6.99. I also give United Service Dog and its agents, permission to contact me via text, phone and email and to give promotional offers periodically.
